Identify low blood sugar signs on scenario cards
When to use this goal
For students with diabetes who are learning to notice and report the signs of low blood sugar listed in their diabetes plan. Present levels and the nurse's teaching plan decide fit. The signs on the cards come from the student's own plan, since signs differ from person to person.
- Baseline
- The nurse builds a set of 10 to 12 cards using the signs in the student's plan (shaky, sweaty, hungry, cranky, hard to think) mixed with ordinary events, and scores the first two sessions. Note which signs the student misses.
- Tips
- Knowledge cards are the first step; real reporting is the goal that matters (fn-hlt-027). Use the student's own words for signs. Ask the family to report signs they notice at home so the cards stay accurate.
- Limits
- Sorting cards does not show the student will notice signs in their own body during class; pair with a reporting goal and the nurse's checks. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Goal
By the annual IEP review date, given picture or written scenario cards showing the low blood sugar signs from their diabetes plan mixed with everyday events, the student will sort each scenario card into "tell an adult now, could be low" or "everyday event" independently with at least 90% accuracy on scenario cards, in 3 of 4 consecutive weekly probes, as measured by opportunity-by-opportunity data collected weekly with the nurse.
Baseline: 50% accuracy.

Met: Observable behavior. Stated as an action.
Met: Condition. Stated.
Met: Unit fits the behavior. Percent accuracy fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Opportunity (trial) recording gives percent accuracy data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Health & Self-Management.
Met: Level of support. Defined.
Met: Blanks filled in. Nothing left to fill in.
Checks structure and wording only. Whether the goal fits the student is the team's call.
Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
