Why log missed sessions too
The IEP states the frequency, location, and duration of each service (34 CFR §300.320(a)(7)), and services must be provided as written (34 CFR §300.323(c)(2)). A log that only lists delivered sessions can't show why minutes fell short. Status codes separate student absences from provider or school causes, which is what teams usually need when discussing make-up time.
Medicaid session documentation
Many schools bill Medicaid for covered services. CMS's 2023 guide describes a minimum record for each service: the date, the student and their Medicaid ID, the provider agency and individual provider, the nature and extent of the service, and where it took place. States often add more, such as codes, credentials, or signatures. Schools also need written parental consent before first billing a child's public benefits (34 CFR §300.154(d)(2)(iv)).
IEP Atlas provides a blank paper form only. It doesn't collect Medicaid IDs, submit claims, or replace your district's billing system. Check required fields with your Medicaid coordinator.
