Free progress-monitoring data sheets
Blank, printable data sheets for the common ways IEP goals are measured. Each one comes as a print-ready page, a PDF, and a spreadsheet with formulas. To get a sheet with your goal's exact criterion, steps, or rubric already filled in, build the goal first.
Accuracy tracker
Number correct out of number attempted, with percent accuracy and support level.
Opportunity tracker
Records each opportunity as independent, prompted, or not demonstrated.
Frequency & rate tracker
Counts a defined behavior during a fixed observation period. Calculates rate when time is recorded.
Duration tracker
Times how long a behavior lasts and compares it with the target duration.
Latency tracker
Times the gap between a cue and the start of the response.
Rubric tracker
Scores each rubric criterion and totals the score for each sample.
Task-analysis checklist
Marks each step of a routine as independent, with support, or not completed across sessions.
Reading fluency tracker
Words attempted, errors, words correct per minute, and accuracy for each probe.
Choosing the right sheet
The sheet should match how the goal's criterion is measured. Otherwise the data you collect can't show whether the goal was met.
- “With 85% accuracy” → accuracy tracker
- “In 4 of 5 opportunities” → opportunity tracker
- “No more than 2 times per class period” → frequency tracker
- “For at least 15 minutes” → duration tracker
- “Within 60 seconds of the direction” → latency tracker
- “A score of 9 out of 12 on a rubric” → rubric tracker
- “8 of 10 steps independently” → task-analysis checklist
- “90 words correct per minute” → reading fluency tracker
IEP Atlas sheets are blank forms. IEP Atlas doesn't collect or store student performance data. Record data only in systems your school approves.
