Free opportunity (trial-by-trial) data sheet

Use this sheet for goals written as a number of opportunities, such as “in 4 of 5 opportunities.” Each box is one opportunity, marked independent (+), prompted (P), or not demonstrated (−).

When to use this sheet

  • Requesting help, self-advocacy, greetings, or using a strategy when a defined situation occurs
  • Following directions, where each direction given is one opportunity
  • Planned trials during instruction

How to use it

  1. Define what counts as an opportunity before collecting data, and write the definition on the sheet.
  2. Mark NA when no opportunity occurred, so missing data isn't counted as failure.
  3. Count + marks toward the criterion unless the goal counts prompted responses.

Questions

What if fewer opportunities occur than the goal requires?

Record what happened and note it. Many teams plan contrived opportunities so the required number occurs. A session with too few opportunities usually shouldn't count toward mastery.

Should prompted responses count?

Only if the goal says so. Recording prompts separately shows whether independence is growing, which is often the real target.
