Free task-analysis data sheet

Use this sheet for routines broken into steps, such as arrival, packing up, handwashing, or a problem-solving sequence. Each step is marked I (independent), S (with support, noting the prompt), or N (not completed), with one column per session.

When to use this sheet

  • Functional and daily-living routines
  • Organization routines (materials, binder, locker)
  • Multi-step academic procedures

How to use it

  1. Write each step so an observer can see whether it happened.
  2. Use the least intrusive prompt first, and record the prompt actually used.
  3. Total the I marks per session. The goal's criterion counts independent steps.

Questions

How do I add my own steps?

Open a task-analysis template in the builder and edit, add, reorder, or remove steps. The sheet updates to match.
