Free accuracy data sheet (percent correct)

Use this sheet when a goal is scored as percent correct on a defined set of items, such as math problems, spelling words, or comprehension questions. The same layout works for work-completion goals (items completed out of items assigned).

When to use this sheet

  • Computation, spelling, vocabulary, or comprehension goals scored item by item
  • Work-completion goals (completed ÷ assigned)
  • Any goal where the denominator is the same kind of item every session

How to use it

  1. Decide in advance whether the denominator is items attempted or a fixed item set, and keep it the same.
  2. Record the support actually provided, even when the goal calls for independent performance.
  3. Percent = number correct ÷ number attempted × 100. The CSV calculates this for you.

Questions

Should I use attempted items or total items as the denominator?

Use whatever the goal specifies, and keep it consistent. If the goal says “given 10 problems,” use 10. If the student is allowed to skip items, attempted items show accuracy, and a separate completion measure may also be needed.

Is percent accuracy right for behavior goals?

Usually not. Percentages need a clear denominator. For staying engaged, use duration. For starting work, use latency. For countable events, use frequency or opportunities.
