Find local services in a resource guide
When to use this goal
For young adults who will need community services (a clinic, a food pantry, a job center, a library) and do not know how to find them. Uses a printed or online local guide such as a 211 site; present levels in reading decide fit, not grade.
- Baseline
- Score each lookup as complete (right service type, address, hours, phone) or not. Use services near the student's home.
- Tips
- Visit one service the student finds. Pair with a goal for calling or visiting.
- Limits
- Guides go out of date; teach calling ahead. Adult disability services are covered in transition skills.

Goal
By the annual IEP review date, given the local community resource guide or a 211 website and 6 to 8 need cards (a free clinic, a job center, a food pantry near home), the student will find a service for each need and record its address, hours, and phone number independently with at least 85% accuracy on service lookups, in 3 of 4 consecutive biweekly probes, as measured by work-sample analysis collected every two weeks.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
