Request missing braille materials from teachers

9-12, 6-8OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For older braille users who are ready to take on part of getting their own materials, as they will in college and work. Present levels in self-advocacy and in knowing what each class needs decide fit.

Baseline
Keep a log for a month of times materials were missing and whether the student asked, waited for the TVI to notice, or did nothing. Teachers can report unrequested gaps.
Tips
The adults still own getting materials ready on time; this goal is about the student's voice, not shifting the duty. Praise and honor each request.
Limits
The count depends on how often materials are missing, which should be rare. The template does not replace the school's responsibility for accessible materials.

Original IEP Atlas template based on general measurement principles; not copied from any goal bank. ID ac-brl-034.

Goal

By the annual IEP review date, when a handout, test, or reading arrives in print only or is missing in a general education class, the student will tell the teacher which item is missing and ask for it in braille or a digital file by a set date independently in at least 4 of 5 opportunities, across 2 consecutive grading periods, as measured by frequency recording collected weekly from a request log.

Baseline: 1 of 5 opportunities.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.
