Answer questions about body changes in puberty

3-5, 6-8Percent accuracyTemplateReview and individualize before use. How templates are made
When to use this goal

For students in grade 4 to 6 puberty lessons who need a slower pace, more pictures, or repeated review to answer questions about the changes ahead. Present levels and family wishes, not grade, decide fit.

Baseline
Ask the questions privately before the unit starts to see what is already known. Record the kind of support needed, such as pictures or read-aloud. Share the topic list and materials with the family ahead of time, and follow the district's health curriculum and any family opt-out choices.
Tips
Score each question as one item. Keep the tone matter of fact and private. Questions the student asks that go beyond the lesson are welcome and are passed to the family or nurse as agreed.
Limits
Knowledge checks do not show comfort or readiness, which are family conversations. Not a sexuality education goal; follow the district curriculum and family choices.

Goal

By the annual IEP review date, after a puberty lesson, given 8 picture-supported questions about body changes and hygiene, the student will answer each question about body changes and the hygiene step that goes with them independently with at least 75% accuracy on puberty questions, in 3 of 4 consecutive weekly probes, as measured by opportunity-by-opportunity data collected weekly during the unit, then monthly review.
