Review a completed application form for errors
When to use this goal
For older students preparing for work or postsecondary applications who fill in forms but leave fields blank, put information in the wrong field, or misspell key details. Present levels decide the length of the form and whether it is paper or online.
- Baseline
- Have the student fill in and review a practice form with no coaching. Score each field after the review. Separately count errors the student caught, which shows whether the review itself is happening.
- Tips
- Use real-looking forms (job applications, program sign-ups, a medical history page) without any company branding. The personal information card holds the student's own details so the goal is checking, not recall. Score from the finished form.
- Limits
- Fields that ask for information the student does not have (a reference's phone number) are not checking errors. Reading level of the form affects the score; choose forms the student can read with their usual supports.

Goal
By the annual IEP review date, after filling in a practice job or program application with 15 to 20 fields, the student will check each field against the personal information card and the field label, and correct blank, misplaced, or misspelled entries independently with at least 95% accuracy on form fields that are complete and correct after the student's review, on 3 of 4 consecutive practice forms, as measured by work-sample analysis collected every two weeks.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
