Wear a T-shirt with the tag at the back

K-2, 3-5OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For students who put on a pullover shirt without help but often end up with it backward or inside out. Fits when the student can already get the shirt on. Present levels and the OT's observation of the routine, not grade or age, decide whether this template fits.

Baseline
Record each shirt change for two weeks: whether the shirt ended up forward and right side out, and whether the student fixed it on their own. Note whether the shirt had a front picture, since that cue is often easier than a tag.
Tips
Teach one orientation method and use it at home and school: lay the shirt face down, tag nearest you, then put the arms in. Score one opportunity for each shirt put on. A discreet inside mark is fine for an older student.
Limits
Does not measure the motor steps of getting the shirt on. Shirts with no tag or print may need an added mark before the goal is fair.

Goal

By the annual IEP review date, when putting on a T-shirt after PE, swimming, or a clothing change, the student will find the tag, turn the shirt so the tag is at the back and inside, and put it on facing forward independently in at least 4 of 5 opportunities, on 3 of 4 consecutive weekly checks, as measured by direct observation collected at each shirt change, summarized weekly.
