Read the key facts on a medicine label
When to use this goal
For older students moving toward managing their own medicine with family support, who cannot yet find the name, dose, times, and warnings on a label. Present levels in reading short phrases, numbers, and times decide fit. Practice uses empty, cleaned containers or printed labels only.
- Baseline
- Use 5 labels and 4 questions each. Separate prescription and over-the-counter labels in the data; their layouts differ.
- Tips
- Plan with the school nurse and family. Teach label words (tablet, take by mouth, every 6 hours, with food, may cause drowsiness). Medication safety routines are in community-safety goals (fn-cs).
- Limits
- Reading a label is not permission to take medicine. This goal never replaces the nurse's plan, the family's oversight, or a doctor's instructions.

Goal
By the annual IEP review date, given 5 printed prescription and over-the-counter labels and 4 questions about each, the student will find the medicine name, the dose, when to take it, and one warning on each label independently with at least 90% accuracy on label questions, across 3 consecutive weekly probes, as measured by work-sample analysis collected weekly.
Baseline: 30% accuracy.

Met: Observable behavior. Stated as an action.
Met: Condition. Stated.
Met: Unit fits the behavior. Percent accuracy fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Work-sample analysis gives percent accuracy data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Functional Academics.
Met: Level of support. Defined.
Met: Blanks filled in. Nothing left to fill in.
Checks structure and wording only. Whether the goal fits the student is the team's call.
Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
