Complete the personal section of a job application
When to use this goal
For older students applying for jobs who leave fields blank or put information in the wrong place. Present levels in writing or typing and in personal information recall decide fit. Students can use a personal information card or a saved master application.
- Baseline
- Score 10 fields on 2 applications. Note fields left blank, put in the wrong place, or answered with the wrong information (mailing versus home address).
- Tips
- The master sheet holds name, contact, availability, references, and work history; build it with the student. Teach that some fields (date of birth, disability) may be optional or not allowed on applications. Keep the sheet private.
- Limits
- Does not measure the work history or short-answer sections. Online applications often time out; practice saving progress.

Met: Observable behavior. Stated as an action.
Met: Condition. Stated.
Met: Unit fits the behavior. Percent accuracy fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Work-sample analysis gives percent accuracy data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Functional Academics.
Met: Level of support. Defined.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
