Tell an adult at a seizure warning sign
When to use this goal
For students with epilepsy who have a warning (an aura or other early sign named in their seizure action plan) and who can learn to tell an adult and get to a safe position. Base the choice on the seizure action plan, the family, and the nurse. Not for students whose seizures come without warning.
- Baseline
- Review the seizure log with the nurse and family: how many warnings the student had, whether they told an adult, and how long it took. Practice runs give data between real events.
- Tips
- Every opportunity counts; telling and getting safe protect the student. Adults follow the seizure action plan from the moment they are told. Choose a signal that works even if speech is affected.
- Limits
- Warning signs differ by seizure and may not come every time; the goal covers only seizures with a warning. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Goal
By the annual IEP review date, when they notice a warning sign named in their seizure action plan during class, the student will tell the nearest adult using their agreed words or signal and sit or lie down in a safe place independently in 5 of 5 opportunities, across 2 consecutive grading periods, as measured by frequency recording collected after each warning and monthly practice.
Baseline: 2 of 5 opportunities.

Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Record up to 5 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
