Shift weight or ask for a position change

When to use this goal

For students who use a wheelchair or sit for long periods and whose health or PT plan calls for regular pressure relief, but who wait for an adult to remember. Base the choice on the PT's and nurse's plan, which sets the method and the schedule.

Baseline
For a week, record each scheduled relief time: whether the student did it, asked for help, or waited for an adult. The PT checks technique once before data start.
Tips
Score each scheduled time as met when the student started the relief or asked within a short wait. Teachers allow the movement at any time. Skin checks and equipment belong to the nurse and PT.
Limits
Does not check that the relief is done well enough; the PT reviews that. Skin problems need medical attention right away. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Something an adult can see, count or time.

Criterion

Number of opportunities in which the behavior is demonstrated, out of a fixed number.

Sample numbers. Replace them with the student's data.

Criterion wording

Data collection

An adult observes and records the defined behavior during a specified activity.

How often data are collected
Mastery rule

How many measurements must meet the criterion.

Goal

By the annual IEP review date, when their pressure relief reminder sounds during class, the student will complete their taught pressure relief (a lean, a push-up, or a tilt) for the time the PT set, or ask an adult for a position change independently in at least 5 of 6 opportunities, on 4 of 5 observed days, as measured by direct observation collected daily.

Baseline: 1 of 6 opportunities.

Measurability check11 of 11 met
  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Direct observation gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Health & Self-Management.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.

Short-term objectives (optional)

When are objectives required?

Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.
