Check in for a clinic appointment with ID
When to use this goal
For high school and transition students who go to medical or dental appointments but leave the check-in to a parent. Present levels decide fit. Practice can happen on community outings with mock check-ins, then at real appointments with the family.
- Baseline
- Run a mock check-in and ask the family to describe the last real one. Make sure the student has an ID card or the school's ID and a copy of the insurance card.
- Tips
- Family members record real check-ins with a simple checklist. The student keeps cards in one place in a wallet or phone case. Pair with the appointment-scheduling goal in independent living.
- Limits
- Clinic forms and procedures vary; mock check-ins should copy local forms. Privacy at the desk matters; teach the student to keep voice low for health details. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

- Total the number of I marks for the session. The goal counts steps completed independently (target: 5 of 6).
- Use the least intrusive prompt first and record the prompt actually needed.
