Log seizures with family in a shared tracker

9-12, 6-8OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For older students who have seizures and are starting to take part in tracking them for their doctor visits. Present levels in using a phone app or paper log decide fit. The student logs with a family member or nurse; it is never their job alone to notice or record a seizure they could not see.

Baseline
Ask the family how seizures are logged now and who does it. Review the last month's log and note which details are usually missing. Count an opportunity as met when the student made or completed the entry within a day.
Tips
Use the tracker the family and doctor already use (an app, a paper log, a shared note). The family or nurse fills in what the student did not see; the student adds how they felt before and after. Keep the log private.
Limits
Seizure frequency varies, so some months have few opportunities. The student often cannot recall a seizure, so accuracy depends on the adult who saw it. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Something an adult can see, count or time.

Criterion

Number of opportunities in which the behavior is demonstrated, out of a fixed number.

Sample numbers. Replace them with the student's data.

Criterion wording

Data collection

Each occurrence of a clearly defined behavior is tallied during a fixed observation period.

How often data are collected
Mastery rule

How many measurements must meet the criterion.

Goal

By the annual IEP review date, after a seizure or warning sign at home, with a family member who saw it, the student will enter the date, time, length, and what happened before in the shared seizure tracker independently in at least 4 of 5 opportunities, across 2 consecutive grading periods, as measured by frequency recording collected monthly review of the tracker with the family and nurse.

Baseline: 1 of 5 opportunities.

Measurability check11 of 11 met
  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Frequency (event) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Health & Self-Management.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.

Short-term objectives (optional)

When are objectives required?

Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.
