Walk straight back to class from the nurse
When to use this goal
For upper elementary students who take a long route, stop to visit other rooms, or go to the office after a nurse visit. Present levels decide fit, not grade. Only for visits the nurse has cleared the student to return from on their own.
- Baseline
- Use the pass log for two to three weeks: the time the nurse sent the student back and the time they arrived. Ask staff along the route where the student tends to stop.
- Tips
- Score an opportunity as met when the arrival time fits the route and the pass reaches the teacher. The nurse and teacher share one simple log. Health needs always come first; a student who feels unwell on the way should return to the nurse.
- Limits
- The log shows timing, not the route taken. Visits for a health plan are not a behavior concern, and the goal should never discourage needed nurse visits.

Goal
By the annual IEP review date, when the nurse clears them to return to class after a visit, the student will walk back to class by the planned route and hand the signed pass to the teacher independently in at least 5 of 6 opportunities, across 3 consecutive weeks, as measured by direct observation collected weekly, from the nurse and teacher pass log.

- Record up to 6 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
