Complete a practice training or college application
When to use this goal
For students who plan to apply to a training program, community college, or adult program and can complete simple forms with a personal information card (see fn-tr-003). Present levels in reading forms and recalling own education history decide which sections to include.
- Baseline
- Score 2 practice applications in different layouts. A field is correct when the right information is in the right place and readable. Separate recall errors (graduation year) from placement errors, which need different teaching.
- Tips
- Use fictional practice forms until the real application, and never submit practice forms. Online forms add navigation steps (next buttons, required-field errors); practice those on a classroom simulation. The real application is completed with the family and the program's access office when accommodations are requested.
- Limits
- Practice forms are simpler than real portals with accounts and uploads. Essays and references are separate writing and self-advocacy tasks.

Goal
By the annual IEP review date, given a practice application for a training program with personal, education, and program-choice sections, and a personal information card, the student will complete the required sections of the practice application independently with at least 90% accuracy on required application fields, on 3 consecutive practice applications, as measured by work-sample analysis collected every two weeks.
Baseline: 45% accuracy.

Met: Observable behavior. Stated as an action.
Met: Condition. Stated.
Met: Unit fits the behavior. Percent accuracy fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Work-sample analysis gives percent accuracy data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Transition-Related Skills.
Met: Level of support. Defined.
Met: Blanks filled in. Nothing left to fill in.
Checks structure and wording only. Whether the goal fits the student is the team's call.
Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
