Assemble an exit transition portfolio
When to use this goal
For students in their final year who will need key documents and contacts after school but have them scattered across adults and folders. Present levels in organization decide whether the portfolio is a binder or a secure digital folder.
- Baseline
- Check what the student can gather in the first month. Record which items exist but are held by others; getting copies is part of fn-tr-035.
- Tips
- Never store identity documents or Social Security numbers in a school portfolio; list where the family keeps them. The portfolio goes home with the student at exit.
- Limits
- A complete portfolio is only useful if the student uses it; ask them to find items during checks.
