Fill in a form legibly with personal information

When to use this goal

Fits secondary students preparing for work, community, or post-secondary settings who cannot complete a paper form (name, address, phone, date of birth, signature) so that it can be read, or who write outside the fields. Base the choice on a trial on a practice form during baseline.

Baseline
Give two practice forms and score each field for correct content, legibility, and placement. Separate the three error types; wrong content means the student needs the reference card, illegible means letter size and formation, and out-of-bounds means spatial planning. Check whether the student knows their own address and phone number.
Tips
Use real school forms when they come up and practice job applications otherwise. The wallet card is a lasting accommodation many adults use; the goal can keep it as the default support. Pair with transition-related goals on personal information and signatures.
Limits
Online forms are now common and need keyboarding, not handwriting. Form layouts vary; score only forms with similar field sizes. Memorizing personal information is a separate skill in functional academics.

Goal

By the annual IEP review date, given a practice job application with 10 fields and the student's information on a reference card, the student will write the correct information in each field, legibly and within the field boundaries independently with at least 90% accuracy on fields completed correctly, legibly, and within the boundaries, on 3 consecutive weekly practice forms, as measured by work-sample analysis collected weekly on a practice or real form.

Baseline: 40% accuracy.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Percent accuracy fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Work-sample analysis gives percent accuracy data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Handwriting.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
