Shift weight in the wheelchair for pressure relief
When to use this goal
For full-time wheelchair users with limited sensation or movement whose plan calls for regular weight shifts, and who now do them only when an adult reminds or helps. The medical and PT plan, not grade, sets the method and timing.
- Baseline
- List the planned weight-shift times for one day and record for a week whether each happened, was prompted, or was missed. Check with the nurse on how pressure areas are monitored; they track the outcome.
- Tips
- This is a lifelong health habit; owning it at school prepares the student for adult life. Discreet alerts suit older students who do not want adults reminding them in front of peers.
- Limits
- The health plan governs; this goal never replaces nursing checks of skin. A student who cannot shift alone needs a staff-assisted or tilt-in-space routine instead. The goal records doing the shift, not whether it was long enough unless timed.

Goal
By the annual IEP review date, at each weight-shift time set in the health plan during class, the student will do the weight shift the PT taught (forward lean, side lean or push-up) and hold it for the time set in the plan with no adult reminder in at least 5 of 6 opportunities, on 4 of 5 school days, as measured by frequency recording collected daily.
Baseline: 2 of 6 opportunities.

Met: Observable behavior. Stated as an action.
Met: Condition. Stated.
Met: Unit fits the behavior. Opportunities fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Frequency (event) recording gives opportunities data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Functional Mobility.
Met: Level of support. Defined.
Met: Blanks filled in. Nothing left to fill in.
Checks structure and wording only. Whether the goal fits the student is the team's call.
Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Record up to 6 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
