Read their own name and personal information words

When to use this goal

For students who need to recognize the words that label their own information on forms, lockers, schedules, and ID cards. Base the choice on the team's functional priorities; it is written for young students learning to find their name and for transition-age students completing forms.

Baseline
Pull the labels from forms the student will actually meet (school registration, library card, job application, medical intake). Score reading the label and knowing what belongs there separately. For young students, start with the name-card condition and note whether the student picks by first letter only.
Tips
Pair with a writing goal for filling in the same fields. For transition-age students, use real applications and intake forms with the personal information covered. Keep the set to words the student will meet repeatedly.
Limits
Reading a label does not mean the student can supply the information; measure that separately. Forms vary in wording (surname, family name, last name), so include common variants. Not a general sight-word measure.

Goal

By the annual IEP review date, given 15 printed personal information words and labels (first name, last name, address, phone, birthday, school, grade, teacher, parent, emergency contact), the student will read each word or label aloud and say what information goes with it independently with at least 90% accuracy on personal information words, across 3 consecutive sessions, as measured by opportunity-by-opportunity data collected weekly.

Baseline: 30% accuracy.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Percent accuracy fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Opportunity (trial) recording gives percent accuracy data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Sight Word Recognition.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.

Short-term objectives (optional)

When are objectives required?

Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.
