Read the words on common forms and product labels
When to use this goal
For high school students whose transition plan includes independent living or employment and who cannot read the recurring words on forms, prescription labels, and household products. Base the choice on the transition assessment.
- Baseline
- Collect the actual forms and labels the student will meet (job application, lease, medication bottle, cleaning product) and pull the recurring words. Score reading and meaning separately. Note which words the student decodes and which need to be taught as whole units.
- Tips
- Use real documents with personal details removed. Pair with a functional writing goal for completing the same forms and with a safety goal for acting on warning labels. A personal glossary the student keeps is a reasonable long-term support.
- Limits
- Form wording varies; teach common variants. Reading a warning does not mean the student will follow it; measure the action separately if safety is the concern. Label print is often very small, which is an access issue rather than a reading one.

Goal
By the annual IEP review date, given 20 words and phrases that recur on forms (signature, date of birth, employer, emergency contact, print, check one), the student will read each word or phrase aloud and say what it is asking for or telling the reader independently with at least 90% accuracy on form and label words, across 3 consecutive sessions, as measured by opportunity-by-opportunity data collected weekly.
Baseline: 35% accuracy.

Met: Observable behavior. Stated as an action.
Met: Condition. Stated.
Met: Unit fits the behavior. Percent accuracy fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Opportunity (trial) recording gives percent accuracy data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Sight Word Recognition.
Met: Level of support. Defined.
Met: Blanks filled in. Nothing left to fill in.
Checks structure and wording only. Whether the goal fits the student is the team's call.
Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
