Answer questions about personal information
When to use this goal
For students who cannot state their full name, age, birthday, school, address, or phone number when asked, which matters for safety and independence. Base the choice on family input; the set of items grows with age (name and age for young children, address and emergency contact for older students).
- Baseline
- With the family, decide which items the student should know (and which must stay private) and probe each with a familiar and then an unfamiliar adult. Record whether the student gives the item, a partial answer, or nothing. For AAC users, confirm the items are programmed.
- Tips
- List the items in the condition; score each correct when it is accurate and complete (full name, not first name only). Teach when it is safe to share information and with whom; the role-play condition should include practice declining to answer a stranger. A card is an acceptable long-term accommodation for some students; record which support was used.
- Limits
- This is memorized information; it does not measure broader expressive language. Family privacy choices decide the item set, so the goal is not comparable across students. Some items (phone numbers) are long and may need a card permanently.

Goal
By the annual IEP review date, when asked 5 personal information questions by a familiar adult (such as full name, age, birthday, school, teacher), the student will state the correct personal information for each question using speech, sign, or a communication device independently with 100% accuracy on personal information questions, across 3 consecutive weekly probes, as measured by opportunity-by-opportunity data collected weekly.
Baseline: 40% accuracy.

Met: Observable behavior. Stated as an action.
Met: Condition. Stated.
Met: Unit fits the behavior. Percent accuracy fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Opportunity (trial) recording gives percent accuracy data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Expressive Language.
Met: Level of support. Defined.
Met: Blanks filled in. Nothing left to fill in.
Checks structure and wording only. Whether the goal fits the student is the team's call.
Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
