Fill out a direct deposit form

9-12Task-analysis stepsTemplateReview and individualize before use. How templates are made
When to use this goal

For students starting a paid job or work program who are handed a direct deposit form. Needs copying numbers from a sample check or bank letter; present levels decide fit, not grade.

Baseline
Run the routine at least 3 times with the same materials before setting a target. Mark each step independent, prompted (note the prompt), or not done, and count only independent steps. Practice uses sample documents and practice or demo accounts only; the student's real PIN, passwords, and account numbers are never used or written down at school.
Tips
Teach that account numbers go only on forms from the employer, never in reply to a text or email.
Limits
The real form is completed with the family or job coach. Practice uses sample documents and practice or demo accounts only; the student's real PIN, passwords, and account numbers are never used or written down at school.
