How to measure IEP goals
Pick the measure from the behavior: scored items take percent accuracy, chances to use a skill take opportunities, countable events take frequency, and routines take a task analysis. Each page explains one method, shows goals that use it, and links the data sheet that records it.
Percent Accuracy
Percent accuracy answers one question: of the items the student tried, how many were correct? It fits skills with clear right and wrong answers, like words read, problems solved, or questions answered.

Opportunity (Trial) Data
Opportunity data answers: when the chance came up, did the student use the skill? Each opportunity is recorded as independent, prompted, or not demonstrated, and the criterion is a count, such as 4 of 5 opportunities.

Frequency Recording
Frequency recording answers: how many times did the behavior happen in a set period? It counts discrete events, like comments made, questions asked, or breaks requested, during a defined time.

Rate per Minute
Rate answers: how many correct responses can the student give per minute on a timed probe? It shows fluency, meaning speed and accuracy together, for skills other than oral reading, such as math facts or letter sounds.

Duration Recording
Duration recording answers: how long did the behavior last? It times a behavior from start to end, such as minutes engaged in work, minutes of independent play, or how long a routine takes.

Latency Recording
Latency recording answers: how long after a cue did the student start? It times the gap between a direction, signal, or event and the start of the response.

Rubric Scoring
Rubric scoring answers: how well did the student do on each part of a complex task? Each row of the rubric scores one part, and the total shows overall quality.

Task Analysis
Task analysis answers: which steps of a routine can the student do on their own? The routine is broken into ordered steps, and each step is marked independent, with support, or not done.

Oral Reading Fluency (WCPM)
Oral reading fluency answers: how many words can the student read correctly in one minute from an unpracticed passage? It is reported as words correct per minute (WCPM) and is used only for reading connected text aloud.

Work Completion
Work completion answers: what share of assigned work did the student finish and turn in? It counts assignments or items completed out of those assigned, without scoring whether answers were right.
