Request AT accommodations for college or testing

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For high school juniors, seniors, and transition-age students who will request AT accommodations from colleges, testing agencies, training programs, or employers. Present levels in knowing their accommodations decide fit.

Baseline
List upcoming requests and record who completed each. Use mock requests if real ones are few.
Tips
Deadlines for testing accommodations can be months ahead; plan early. Colleges do not have to use the IEP, so documentation matters.
Limits
Approval is decided by the outside agency. The AT specialist and team choose the tool from an AT assessment and trial data; the template measures what the student does with it, not the tool's quality.

Original IEP Atlas template based on general measurement principles; not copied from any goal bank. ID ac-at-034.

Goal

By the annual IEP review date, when preparing for a college entrance exam, a college application, or a job training program, the student will find the request process, gather the needed documents, and submit a request that names their AT and why it is needed independently in at least 2 of 3 opportunities, across 2 consecutive semesters, as measured by frequency recording collected monthly.

Baseline: 0 of 3 opportunities.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Frequency (event) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Assistive Technology Use.

  • Met: Level of support. Defined.
