Call the staff contact when separated

When to use this goal

For transition-age students who are gaining independence on outings, job sites, or public transit and need a plan for getting separated from staff. Fit depends on phone use skills and community instruction data, not grade. Practice is done through planned, supervised separations and role-plays.

Baseline
Start with role-plays at school: describe a separation and ask the student to show what they would do with their own phone. Score staying in place, contacting staff, and describing the location. Check the phone is charged and the contact is saved before every outing.
Tips
Describing the location is the hard part; teach the student to read a nearby sign or store name aloud. Agree with the family on the contact number. Safety skills are targeted at every opportunity.
Limits
Practice separations need a written plan and staff in sight at all times. This goal does not replace the outing safety plan or a family's own safety arrangements.

Goal

By the annual IEP review date, during a planned practice separation on a community outing with staff watching from a distance, the student will stay in place, call or text the staff contact saved in the phone, and say where they are independently in 4 of 4 opportunities, across 3 consecutive monthly checks, as measured by opportunity-by-opportunity data collected four planned practices or role-plays per month.

Baseline: 1 of 4 opportunities.

Measurability check11 of 11 met
  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Opportunity (trial) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Safety Behaviors.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
