Fill in a simple form with name, date, and requested information

When to use this goal

For students who can write or type their name but leave sign-in sheets, permission slips, and order forms blank or put information in the wrong boxes. Present levels in writing and reading field labels decide which fields to include; for job applications, use fn-tr-003.

Baseline
Score 2 or 3 forms in different layouts. A field is correct when the right information is in the right box and readable. Date fields are the most common error; note the format the student uses.
Tips
Use real classroom forms (sign-in, lunch count, book order) so the skill has daily opportunities. Teach the date format the school uses. Keep completed forms with personal information in the classroom.
Limits
Forms with free-response fields (reason for visit) need writing skills beyond this template. Online forms add required-field errors and navigation that may need their own progression.

Goal

By the annual IEP review date, given a simple one-page form (sign-in sheet, library card form, field trip slip, lunch order form) with 6 to 10 labeled fields, the student will complete each field on the form with the correct information independently with at least 90% accuracy on form fields, on 3 consecutive forms, as measured by work-sample analysis collected weekly.

Baseline: 40% accuracy.

Measurability check11 of 11 met
  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Percent accuracy fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Work-sample analysis gives percent accuracy data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Functional Academics.

  • Met: Level of support. Defined.
