Sign name on the signature line

6-8, 9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For students who print their name but do not have a consistent signature or do not find the signature line on forms. Present levels in writing their name decide fit. A signature can be printed, cursive, or an adapted mark the team documents; consistency matters more than style.

Baseline
Collect 8 forms or sheets and check that the signature is on the right line and matches the agreed signature. Keep a sample of the agreed signature for comparison.
Tips
Choose the signature with the student and family. A stamp or adapted mark may be the student's signature; record it. Pair with fn-fa-058 so the student knows what they are signing.
Limits
Legal signing of contracts and consent for adults involves capacity and decision-making support; that is a transition planning topic beyond this goal.

Original IEP Atlas template based on general measurement principles; not copied from any goal bank. ID fn-fa-040.

Goal

By the annual IEP review date, given a form with name, date, and signature lines, such as a field trip or work permission form, the student will find the signature line and sign the same way each time independently in at least 7 of 8 opportunities, across 3 consecutive weekly checks, as measured by opportunity-by-opportunity data collected weekly, from forms and sign-in sheets.

Baseline: 2 of 8 opportunities.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Opportunity (trial) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Functional Academics.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
