Ask what a document is before signing it

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For older students who sign anything put in front of them. Present levels in asking questions and in reading form titles decide fit. The goal protects the student: find out what the document is and ask a trusted person before signing.

Baseline
In 5 role-plays (a permission form, a phone contract, a work form), record whether the student asked about the document before signing. Count signing without asking as not correct.
Tips
Every opportunity counts because signing the wrong document can cause real harm. Help the student name their trusted people. Link to supported decision-making in the transition plan.
Limits
Role-plays may not show real behavior with pressure from a salesperson. Legal capacity and guardianship are outside this goal.

Goal

By the annual IEP review date, when asked to sign a form at school, a job site, or an appointment, the student will read or ask what the document is and check with a trusted person before signing independently in 5 of 5 opportunities, across 3 consecutive data periods, as measured by opportunity-by-opportunity data collected monthly, during role-plays and with real forms.

  • Met: Level of support. Defined.
