Add an appointment card to a phone calendar
When to use this goal
For older students who get appointment cards or texts (doctor, dentist, job interview, vocational rehabilitation) and lose or forget them. Present levels in reading dates and times and in phone calendar use decide fit.
- Baseline
- Use 6 cards (practice and real) and check the calendar entries for correct date, time, place, and reminder. Count an entry correct only if all four are right.
- Tips
- Let the student choose reminder times. Teach where to find the date and time on different card styles. Health appointments are private; check entries with the student's permission.
- Limits
- Does not measure getting to the appointment. Requires a phone the student can use.

- Record up to 6 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
