Rate pain on a faces scale when asked
When to use this goal
For students who say only "it hurts" or "I'm fine" when asked about pain, so adults cannot judge whether a visit to the nurse is needed. Present levels and the nurse's input, not grade, decide fit. Suits students who can point to a picture scale; the nurse chooses the scale the school uses.
- Baseline
- With the nurse, record 5 or more times the student was asked about pain: what they said, whether they used the scale, and whether the rating fit what adults saw (limping, guarding, crying). Practice scenarios count as opportunities but should be labeled.
- Tips
- Score an opportunity as met when the student uses the scale; the number itself is the student's report and is never scored right or wrong. Teach the scale when the student is not in pain. The nurse acts on the rating, not the IEP team.
- Limits
- Pain is personal, so adults cannot check the rating; the goal measures using the scale, not accuracy. Some students under-report pain; adults keep watching for other signs. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Goal
By the annual IEP review date, when an adult or the nurse asks about pain after an injury, a complaint of pain, or a nurse visit, the student will point to or say a number on the 0-to-10 faces pain scale that matches their pain independently in at least 4 of 5 opportunities, across 3 consecutive weeks, as measured by opportunity-by-opportunity data collected at each real or practice opportunity, reviewed weekly.

Met: Condition. Stated.
Met: Unit fits the behavior. Opportunities fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Opportunity (trial) recording gives opportunities data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Health & Self-Management.
Met: Level of support. Defined.
Met: Blanks filled in. Nothing left to fill in.
Checks structure and wording only. Whether the goal fits the student is the team's call.
Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Record up to 5 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
