Take home medication when the phone alarm sounds

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For older students and transition students who take a daily medication at home and whose family wants them to take it on time with less reminding. Base the choice on the family and the prescriber's guidance. Doses are pre-set in an organizer that an adult checks.

Baseline
For two weeks, the family records each dose: taken on time without a reminder, taken after a reminder, or missed. Use the organizer and chart to check.
Tips
Count each scheduled dose as an opportunity; met means taken within the window without a person reminding. The target is every dose, since medications work as prescribed only when taken. Any missed dose follows the prescriber's instructions.
Limits
Self-report and charts can be wrong; the organizer is the best check. The student does not change doses or timing. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.
