Take heat breaks during outdoor work or sports

9-12, 6-8OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For students whose health condition or medication raises heat risk, or who push through heat without resting, during outdoor jobs, sports, or PE. Base the choice on the health plan and the nurse. The plan sets the break schedule and the warning signs.

Baseline
On 3 or more hot days, record each planned break time and each warning sign: whether the student took the break and how long. Ask coaches what they have seen.
Tips
All breaks every time, since heat illness can be serious. Coaches and supervisors agree to the plan in writing. Score each break or warning as one opportunity.
Limits
Warning signs may be missed by the student; adults keep watching. Emergency signs follow the emergency plan. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Something an adult can see, count or time.

Criterion

Number of opportunities in which the behavior is demonstrated, out of a fixed number.

Sample numbers. Replace them with the student's data.

Criterion wording

Data collection

An adult observes and records the defined behavior during a specified activity.

How often data are collected
Mastery rule

How many measurements must meet the criterion.

Goal

By the annual IEP review date, during outdoor work-based learning, sports practice, or PE on days above the plan's temperature, the student will tell the adult in charge, move to shade, and drink water for the break time in their plan independently in 4 of 4 opportunities, across 3 consecutive months, as measured by direct observation collected on each hot day or warning event.

Baseline: 1 of 4 opportunities.

Measurability check11 of 11 met
  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Direct observation gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Health & Self-Management.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
