Set a weekly activity goal and log progress

When to use this goal

For older students and transition students building a habit of regular physical activity they choose (walking, dancing, swimming, a home workout), who want to track it themselves. Present levels and the student's interests decide fit. Any limits on activity come from the health provider.

Baseline
Ask what activity the student does now and run one planning week. Record which steps they did without help. Check any health limits with the nurse.
Tips
The goal measures the planning habit, not fitness. Let the student choose the activity. Link to leisure and fitness goals where useful.
Limits
Logs are self-report. Students with medical activity limits follow their provider's guidance first. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Criterion

Number of routine steps performed independently.

Sample numbers. Replace them with the student's data.

Task-analysis steps

Write each step so an observer can mark it done or not done.

Data collection

A routine is broken into ordered steps; each step is recorded as independent, supported, or not completed.

Goal

By the annual IEP review date, at the start of each week, during a weekly check-in with a teacher or job coach, the student will set a weekly activity goal, log activity on most days, and compare the total to the goal at the end of the week, performing at least 5 of 6 steps independently, across 4 consecutive weeks, as measured by a task-analysis checklist collected weekly.

Baseline: 1 of 6 steps independently.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Task-analysis steps fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Task-analysis checklist gives task-analysis steps data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Health & Self-Management.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
