Report a possible medicine side effect
When to use this goal
For students who take a medication with side effects their provider asked the family to watch (sleepiness, stomach upset, appetite change, dizziness), and who stay quiet about them. Present levels decide fit. Reporting lets the nurse and family act; the student never stops or changes a medication.
- Baseline
- Ask the family and nurse about recent changes the student noticed or did not report. Run 2 or 3 role-plays to see what the student says.
- Tips
- Score an opportunity as met when the student reported on their own or at the first open question. The side-effect list comes from the prescriber or pharmacy information. The nurse contacts the family as the plan says.
- Limits
- Not every change is a side effect; the goal is reporting, and adults decide. Some changes are noticed first by others. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Goal
By the annual IEP review date, when they notice a change listed on their side-effect card after taking a medication, the student will tell the nurse or a family member what they noticed and when it started independently in 4 of 4 opportunities, across 3 consecutive months, as measured by opportunity-by-opportunity data collected at each check-in after a medication change and monthly role-play.

- Record up to 4 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
