Go to the nurse for scheduled glucose checks
When to use this goal
For students with diabetes who can do parts of a glucose check but rely on the nurse or a teacher to send them at check times. Present levels in reading a schedule or phone alarm and in leaving class on their own decide fit, not grade. The diabetes care plan sets the times and the steps.
- Baseline
- Use the nurse's log for two weeks and count how many checks the student started on time without being sent. Note which check times are missed most (often the one in the middle of a favorite class).
- Tips
- The nurse sets the times and decides what counts as on time. Work with teachers so the student can leave without asking aloud. Reporting the number lets the nurse act on it; the student does not decide treatment alone. Pair with fn-hlt-027 for lows.
- Limits
- A missed check is a health issue first, and the nurse follows up at once regardless of the goal. Students with continuous glucose monitors may check less often, so the opportunity count changes. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Goal
By the annual IEP review date, at each scheduled glucose check time in the school day, the student will go to the nurse or check location at the scheduled time and report the number to the nurse independently in at least 9 of 10 opportunities, across 3 consecutive weeks of data, as measured by frequency recording collected daily, from the nurse's log.
Baseline: 4 of 10 opportunities.

Short-term objectives (optional)
When are objectives required?Required for students on alternate assessments (34 CFR §300.320(a)(2)(ii)); some states require them for everyone.

- Record up to 10 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
