Check out and return shared classroom equipment

3-5, 6-8OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For students who borrow headphones, calculators, or devices and leave them on desks or take them home. Present levels in following a sign-out system decide fit; the class uses a simple check-out chart or numbered slots.

Baseline
For 10 check-outs, record whether the item was signed out and returned to its spot before the student left the room. Note which items are missed; devices that charge in a cart are often left out.
Tips
Number both the item and its spot. Put the return check on the end-of-period routine (fn-rt-038). The same system works in job sites with tool sign-out.
Limits
Measures the student's part only; shared systems also need class-wide routines. Lost items may not be the student's.

Goal

By the annual IEP review date, when the student needs shared headphones, a calculator, or a device, with a numbered check-out chart, the student will sign out the item, use it, and return it to its numbered spot before leaving the room independently in at least 9 of 10 opportunities, across 2 consecutive two-week periods, as measured by opportunity-by-opportunity data collected at each check-out, summarized every two weeks.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Opportunity (trial) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about School-Related Independence.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
