Request time off with enough notice

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For students with a placement or job who miss shifts for planned events without telling the supervisor ahead. Present levels in calendar use and communication decide supports.

Baseline
Log real conflicts over a few months and add practice scenarios. Count a request made by the deadline using the site's method.
Tips
Sites differ (paper form, app, email); teach the real method. Noticing the conflict is often the harder part; pair with calendar use.
Limits
Real conflicts are irregular. The supervisor may deny the request; that does not change the score.

Goal

By the annual IEP review date, when a planned event, such as an appointment, family trip or school event, conflicts with a scheduled shift, the student will request the time off from the supervisor by the site's notice deadline using the site's method independently in at least 4 of 5 opportunities, across 3 consecutive data collection periods, as measured by opportunity-by-opportunity data collected monthly.
