Make a personal signature mark or stamp
When to use this goal
For students with significant motor or support needs for whom a full written signature is not realistic now, who can make a consistent personal mark (an initial, a simple symbol) or use a signature stamp. The student's motor access and what the family and student want, not grade, decide fit.
- Baseline
- Try a few options with the OT (initial, symbol, self-inking stamp) and note which the student can make consistently and prefers. Record five attempts on lines of normal size: on the line and recognizable, off the line, or adult hand-over-hand. Agree the chosen mark with the student and family.
- Tips
- A consistent mark gives the student a way to sign their own work, timesheets and forms rather than an adult signing for them. Keep the stamp or marker with the student's materials. Pair with teaching what each form is for, at the student's level.
- Limits
- Whether a mark or stamp is accepted as a legal signature depends on the setting and local rules; the family and transition team should check for important documents. Measures the motor act, not understanding of what is signed.

- Record up to 5 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
