Fill in practice new-hire forms
When to use this goal
For students starting a first job who have never seen a tax withholding form, a direct deposit form or an emergency contact form. Present levels in reading forms decide whether forms are simplified for practice.
- Baseline
- Score one practice set. A field is correct when the right information is in the right box. Note fields left blank because the student did not know the answer; marking those to ask about is a correct step.
- Tips
- Use fictional forms and personas for practice; real forms are completed with the family. Tax choices on real forms are a family decision, and staff give no tax advice.
- Limits
- Practice forms differ from employer systems, which are often online. Accuracy on forms does not show the student understands withholding or banking.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
