Keep a daily work log on the job

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For transition-age students in work-based learning or a community job who are building functional writing skills, including students with significant support needs. Present levels should describe the student's writing mode (handwriting, typing, word bank, device). Fits when a supervisor or job coach wants a record of tasks.

Baseline
For eight shifts, collect the logs and record which sections were filled in and with what support. Count a log as met when the date, two tasks, and a note are all present and readable by the supervisor.
Tips
Ask the supervisor to read the note and respond, so the writing has a real reader. Word banks should use the job's own task names. The log can later grow into a weekly summary or a timesheet.
Limits
Shift schedules control how often data is collected. Job coaches may fill in sections without meaning to; train them on the support levels. The goal does not measure spelling or handwriting.

Goal

By the annual IEP review date, at the end of a work-based learning shift, given a work log form with spaces for date, tasks, and a note, the student will fill in the work log with the date, at least two tasks completed in words, and one note for the supervisor (such as supplies needed or a question) independently in at least 7 of 8 opportunities, across 4 consecutive weeks of shifts, as measured by opportunity-by-opportunity data collected after each shift, summarized weekly.

Measurability check11 of 11 met
  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Opportunity (trial) recording gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Written Expression.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.

Short-term objectives (optional)
