Keep a transition folder and produce a requested document
When to use this goal
For transition-age students whose present levels show lost schedules, work permits, resumes, and forms needed for jobs, programs, or college visits. For students who are building a transition portfolio.
- Baseline
- Set up the folder with the student and list its contents. Then make several planned requests over two weeks and record whether the document was produced from the folder within 2 minutes. Note whether missing documents were never filed or were removed and not returned.
- Tips
- Each request is one opportunity; planned requests can supplement natural ones. Met means the correct, current document is handed over within 2 minutes. Refile documents after use as part of the routine. A digital folder with the same sections works for students who prefer it.
- Limits
- Requests are infrequent without planning. Does not measure whether the student can fill out forms; see functional academics and transition goals for that.

Goal
By the annual IEP review date, when a staff member, employer, or program asks for a document kept in the transition folder (schedule, resume, work permit, ID copy, application), the student will produce the requested document from the transition folder within 2 minutes independently in at least 4 of 5 opportunities, across 3 consecutive monthly summaries, as measured by opportunity-by-opportunity data collected each request and summarized monthly.

- Record up to 5 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
