Show a medical emergency card when asked

When to use this goal

For students with a health condition or communication need who may not be able to explain their needs in an emergency or when upset, and who carry an emergency card or wear medical ID. Present levels decide fit. The card content comes from the health plan and the family.

Baseline
Run 3 to 5 planned role-plays with different helpers (bus driver, librarian, a staff member the student does not know) and record whether the student found and showed the card. Check that the card is current.
Tips
Keep the card in the same pocket or phone case every day. Score an opportunity as met when the student showed the card within a short wait after being asked. Review the card each semester with the family.
Limits
Role-plays cannot fully copy an emergency. A card helps only if helpers ask or notice it; medical ID jewelry adds a backup. This goal never replaces the individualized health plan, medication orders, or an emergency action plan; the nurse, the family, and the student's health care provider decide care, and the goal follows their plan.

Goal

By the annual IEP review date, when a staff member, bus driver, or community helper asks about their health or who to call, the student will show their medical emergency card or medical ID and point to the contact number independently in 5 of 5 opportunities, on 3 consecutive monthly drills, as measured by opportunity-by-opportunity data collected monthly, at drills and real events.
