Sign full name in cursive on forms
When to use this goal
For older students who need a consistent signature for permission slips, work papers, bank forms, and identification, and who currently print their name or produce a different signature each time. The student's current ability to write the name in cursive decides the starting point.
- Baseline
- Collect five signatures across two days and compare them for consistency and legibility. Decide with the student and family what the signature should look like (full name, first initial and last name); a simplified consistent signature is acceptable for most purposes. Note whether the student fits the signature within the line.
- Tips
- Build the signature into transition activities (job applications, library cards, permission forms) so practice is real. Consistency matters more than beauty; agree on one form and keep the model card until it fades.
- Limits
- Students with significant motor needs may use a signature stamp or an electronic signature as an accommodation; a signature goal is not required for everyone. Does not cover filling in the rest of the form, which has its own template.

- Record up to 5 opportunities per session, matching the goal's denominator.
- Define in advance what counts as an opportunity so every data collector records the same thing.
- Count only + marks toward the criterion unless the goal specifies prompted responses.
