Write name and date legibly on every assignment

When to use this goal

For students who turn in unnamed papers, whose heading cannot be read, or who write the date in a different place each time. Base the choice on a check of a week of turned-in work; this is a low-effort daily habit with a high payoff for lost work.

Baseline
Review two weeks of turned-in work and count papers with a legible name and date in the right place. Separate forgetting (no heading at all) from illegibility (heading present but unreadable); the first calls for a routine, the second for a legibility focus on the few words in the heading.
Tips
Data come from papers already turned in; no probe is needed. Post the date in the same spot daily. For students who still miss the heading after a month, a name stamp is a fair accommodation while the date-writing habit continues.
Limits
Headings are short, so legibility here does not predict legibility elsewhere. Counting only turned-in papers misses assignments never submitted; that is an assignment completion issue.

Goal

By the annual IEP review date, at the start of each written assignment, the student will write the name and date legibly in the heading area before beginning the work independently in 5 of 5 opportunities, across 3 consecutive weekly reviews, as measured by direct observation collected weekly, reviewing five turned-in papers.

Baseline: 2 of 5 opportunities.

  • Met: Observable behavior. Stated as an action.

  • Met: Condition. Stated.

  • Met: Unit fits the behavior. Opportunities fits this behavior.

  • Met: Explicit criterion. Number and unit stated.

  • Met: Mastery rule and schedule. Both stated.

  • Met: Method produces the metric. Direct observation gives opportunities data.

  • Met: Review timeframe. Stated.

  • Met: Baseline and target compatible. Same unit, change in the right direction.

  • Met: Aligned with area of need. Still about Handwriting.

  • Met: Level of support. Defined.

  • Met: Blanks filled in. Nothing left to fill in.

Checks structure and wording only. Whether the goal fits the student is the team's call.
