Take a phone message others can read

9-12OpportunitiesTemplateReview and individualize before use. How templates are made
When to use this goal

For transition-age students in office, reception, or school-based jobs who take phone or in-person messages that coworkers cannot read or that miss key details. Fit depends on job-site samples, not grade.

Baseline
Use five role-played messages and ask a coworker or another adult to read each one back. Score a message as readable when they get every part right.
Tips
Asking the caller to repeat is a real workplace strategy. A typed message on a device is fine where the workplace uses one.
Limits
Mixes listening and handwriting; separate them if the student misses details even when not writing.

Goal

By the annual IEP review date, during a role-play of a phone call with a message to take on a message pad, the student will write the caller's name, number, and message on the message pad so a coworker can read it independently in at least 4 of 5 opportunities, across 3 consecutive weekly checks, as measured by opportunity-by-opportunity data collected five messages per week, real or role-played.

Baseline: 1 of 5 opportunities.
