Submit a time-off request on the work app
When to use this goal
For working students who ask for days off at the last minute or by telling a coworker, at a workplace that uses a paper form or a scheduling app. Present levels in reading forms and using apps, not grade, decide fit.
- Baseline
- Have the student complete one practice request with no guide and mark each field correct or not, including the status check. Ask the employer how far ahead requests are due.
- Tips
- Score each field (dates, start and end times, reason, submit, status check) as correct or not. Use the employer's real form or app screens once the student is placed. This template covers the written request; meeting the notice deadline is in the vocational skills time-off template.
- Limits
- Measures the request itself, not whether time off is approved or whether the student plans ahead. Apps change; update the photo guide when screens change.

Goal
By the annual IEP review date, given the workplace's time-off request form or scheduling app and a planned event at least two weeks away, the student will fill in the dates, shift times and reason on the request, submit it, and check the status a few days later independently with at least 90% accuracy on time-off request fields, on 3 of 4 consecutive requests, as measured by work-sample analysis collected monthly and at each real request.
Baseline: 40% accuracy.

Met: Unit fits the behavior. Percent accuracy fits this behavior.
Met: Explicit criterion. Number and unit stated.
Met: Mastery rule and schedule. Both stated.
Met: Method produces the metric. Work-sample analysis gives percent accuracy data.
Met: Review timeframe. Stated.
Met: Baseline and target compatible. Same unit, change in the right direction.
Met: Aligned with area of need. Still about Employment Foundations.
Met: Level of support. Defined.

- Score only items the student attempted unless the goal defines a fixed set of items. Use the same denominator every session.
- Percent = numerator ÷ denominator × 100. Round to one decimal place.
- Record the support actually provided, even if the goal is independent performance.
